Also, despite the probable long-term scientific and public-health benefit
of this project, I am personally interested only because I would be
working on open source software development.  If we go turnkey, that
mostly ruins it for me.  I also think that we will be doing a better
thing, maybe even in the short term, if we develop some of our own code.

In my experience, off-the-shelf solutions can give you 80% of what you need. And building-your-own can give you 95% (but it will take three times as longer to develop than you think it will).

The quickest customized solution would be an open-source product with
a clean, easily-modified codebase.

Have you looked for an open-source LIMS?  Googling that phrase found
several interesting projects, such as the Plone-based Bika:
