Yes, someone has done this.
 
We're running 4.x on an internal-use-only RHEL 3 AS MySQL server.  I had to recompile PHP, some Perl modules, and some other stuff.  I just did an "rpm -Uvh" on the official MySQL 4 RPMs, noting the requirements that are dependent on MySQL 3, then did an "rpm -Uvh --nodeps" to update the system to the new RPMs.  After the new RPMs were installed, I grabbed the SRPMs for the problem RPMs from RHN and rebuilt them against the MySQL 4.x development package.  Everything works like a charm, but I do have to rebuild the SRPMs if there's ever an update in RHN.  Here's a list of what I had to build:
 
  php
  libdbi
  MyODBC
  perl-DBD-MySQL
 
I created a new sub-channel in our RHN proxy to store the RPMs, to which I subscribe relevant systems.  You might have to rebuild other, additional RPMs since we did a selective install.  Hope this helps.

Hi Folks,

Let me rephrase my question:

Is anyone out there running RHEL with a version of MySQL that is not
officially supported by RedHat?

We've developed a kick a$$ application that depends on MySQL 4.X. Our
servers run something like 3.2.

I'm not a personal fan of RedHat, but it is all that is supported by
our IT folks. They do not want to install any MySQL packages that are
not the official RHEL RPMs. I would like to gather some experiential
data from folks who have installed unsupported MySQL RPMs on their
RedHat systems (I suppose Fedora systems, too).

How did the install go? Do you remember your downtime? How was
backward compatibility? Did you have to tinker around or did things
"automagically" work? Did any PHP scripts break? Don't you love the
union statement in MySQL 4.X?

I yearn for the day when people start to grok that Linux != RedHat.
