Okay, still plenty to try :)
What are the settings for the global AllowOverride directive - both the
global one and the one inside your <virtual> and or <directory>
sections?
Is AccessFileName definitely set to .htaccess
Is AccessConfig set? If so what are the contents of your AccessConfig
file?
