That is one thing that I cannot figure out. But it always is the first
message. I have a theory that the problem only happens after the message
was in the mailq for some reason. However I can't do any extensive testing
when I have 3000 users popping their mail every 3 minutes. ;)

Yes, that was it. They fixed it for us.
Why would a message be missing a from line?
