I don't know a lot about the case, but it looks like it is similar to many 
other cases where the man has an overwhelming need to be in control. 
Once control is taken from him, he may do bizarre things to get back in 
control.  If it is not possible to be in control of the situation, he may 
go so far as to kill himself and others.  In this case, John Alexander was 
sending a clear message to Robert Miller:  You can't tell me what to do. 
He killed Miller's sister (April Wheeler).  I don't know if maybe Miller 
was dating Alexander's estranged wife, but it sounds quite possible.

What could cause this extreme need for control?  I don't know.  We all 
have it to some degree, but for some rare people it takes over their lives 
and becomes seriously pathological.

In this case it is too bad that Miller took matters into his own hands (if 
we can believe some of the stuff in the article) and didn't consult with a 
psychologist/psychiatrist or law enforcement in dealing with Alexander. 
When someone would rather be dead than out of control, you have to either 
give him a sense of control or put him in prison.  You can't taunt or 
threaten that kind of guy!
