Small with low energy usage and very short range signal isn't all that 
difficult to do. (Small with long range would be much harder.) He must 
have had secret service guys near him so it'd be no trick to have a tiny 
transmitter that only had to reach to them. Such a transmitter could be 
small enough to fit inside his ear with the speaker/mic so I don't see 
why he'd have anything on his back. That box on his back would be bigger 
than a cellphone even which would just be silly. And speaking of 
cellphones - you could use similar tech to the better phones of today to 
make the signal of such a device almost impossible to record. Their 
signal uses rotated frequencies and is encrypted. I'd be surprised if 
the President doesn't have access to tools at least that good.. and if 
so why wouldn't he just put it in his pocket like a cellphone? Doing so 
and nobody would even question why he had it. Why would he stick it to 
his back? The whole idea is just silly. When you're hiding things you 
don't make them stick out as unusual.
